Your First Steps

  • Complete and submit the online Patient Information and Registration form. (After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information.)
  • Download additional forms to review prior to your visit: Notice of Privacy Practices and Financial Policy
  • Have a parent or guardian accompany patients under the age of 18.
  • Contact us with questions about insurance and payment. Our practice typically requires that patients pay the fees for the first appointment in full.

Information We May Request

  • A completed Patient Information and Registration form. 
  • A referral form from your dentist. Typically, patients come to our office after their general practitioner has referred them. However, we welcome patients who visit us directly for treatment.
  • X-rays, if applicable. If your general dentist has taken X-rays, please ask them to send these to our practice, or you can bring them yourself.
  • A list of your current medications.
  • Proof of medical or dental insurance if you carry it and would like to apply it to the cost of our services.

Your doctor will listen to what you’d like to accomplish in our care, discuss treatment options, and answer any questions you have. In some cases, the doctor may recommend additional X-rays or other diagnostic tests to provide a more accurate diagnosis and treatment plan.

During your visit, please let your doctor know if:

  • You have a medical condition that may be of concern, such as diabetes, high blood pressure, artificial heart valves and joints, or rheumatic fever.
  • You are currently taking any medications such as heart medicine, aspirin, or anticoagulant therapy.

Union Oral Surgery can perform certain simple procedures on the same day as the initial visit. However, you will likely require a second appointment if you have a complex medical history or need a more in-depth surgical procedure.

We participate with most major dental insurance plans. Please call us if you have questions about your plan and insurance coverage. At your consultation visit, we will discuss your insurance and benefits. Our goal is to help patients maximize insurance reimbursement for covered procedures.  Please understand that estimates of insurance benefits are never guaranteed. Because the insurance policy is an agreement between the patient, their employer and their insurance company, we ask that all patients be responsible directly for all charges.

We deliver the finest care at the most reasonable cost to our patients. Our credit and collection policies are a necessary part of assuring that we have the financial resources needed to maintain this facility for our patients and community. Our collection policies are also essential for allowing us to keep our fees at an affordable level. If there are questions or concerns about our financial and insurance policies, please do not hesitate to contact our business staff at 704.291.7333. Payment is expected when services are rendered. For our patients’ convenience, we accept VISA, MasterCard and Discover.

If extended payment plans are needed, we participate with CareCredit™. Please remember that each patient is fully responsible for all fees charged by this office regardless of insurance coverage. We send monthly statements to our patients who are not paid-in-full. Most insurance companies will respond within four to six weeks after we file with them. Our patients should call our office if their statement does not reflect an insurance payment within the four to six week time frame. Any remaining balance after insurance has paid or denied is the patient’s responsibility. Prompt remittance is expected and appreciated.

Treatments take place in a modern and professional office setting. We have also invested in the latest technology to reduce discomfort, improve outcomes, and speed recovery time.
